Tufts Alumni Awards Nomination Form

Alumni, faculty, staff and friends of Tufts, are welcome to submit nominations for any of the award categories listed below.

The following must accompany this form to ensure that all candidates are judged equally:

  • Nominee’s curriculum vitae (CV) or resume
  • At least one letter of support from one of the nominee’s constituencies (alumni, administration, faculty, staff, or friend of Tufts) stating specific reasons for the nomination

Incomplete nominations will not be considered by the Awards Committee. Additional information (such as articles, letters of support, etc.) may also be submitted.

DEADLINE

Submit nomination forms and other pertinent information by December 1, 2009.
Award nominations are welcome throughout the year preceding the deadline.

Award Category
You may nominate someone for more than one award. Nominees not selected will remain in the nomination pool for one additional year. After that time, the full package must be resubmitted.
